To submit a Final or Interim Impact Report please read the instructions below:

NEW USERS: If you have never accessed our online system, you will use the appropriate link provided below. Once there, you will see the link that says, "First time user? Click here to create your password". Make sure to have your Federal Tax ID number handy, you'll need it. Please be careful not to create a duplicate organization. The system will guide you through the process. Once logged in you will scroll down to the "Applications Requiring Action" area to review your previously approved application and access reports that need to be completed. Please note that the reports will not be visible until approximately 60 days prior to the due date. If you don't see them, please contact the grants manager at the email link below.

ALREADY A USER: If you need to submit an Impact Report and have previously submitted an online application you will simply log back in and scroll down to the "Applications Requiring Action" area to review previously approved applications and access reports that need to be completed. Please note that the reports will not be visible until approximately 60 days prior to the due date. If you don't see them, please contact the grants manager at the email listed below.
